Then comes the part most families only start thinking about in the discharge lounge: what happens at home? Recovery does not end when the paperwork is signed. For the first while you may be moving slower, managing new medications, and finding that ordinary things like stairs, meals, and laundry take more energy than you have. That gap between well enough to leave and back to yourself is exactly where we work.

Getting ready for discharge
We help you prepare for the discharge conversation, stock the fridge, warm up the house, and make sure any equipment like a walker or raised toilet seat is in place before you arrive.
Discharge day
A familiar face picks you up at the hospital, keeps track of your discharge instructions, stops at the pharmacy for prescriptions on the way, and settles you in at home.
The first days home
The riskiest stretch of any recovery. We help with medication reminders, meals, personal care, and keeping an honest eye on how you are really doing, and we keep your family in the loop.
The first weeks
Rides to follow-up appointments, the exercises on your care plan, groceries and housekeeping while your energy comes back. Visits taper off as you recover.
If you live alone
Regular check-ins so someone always knows how you are doing, from a short daily visit up to overnight stays, until you are steady on your own again.

Typical recovery support runs a few weeks and tapers off as you get stronger. If you want us longer, we stay. If you are done early, you stop.
